AJGA Proof-of-Age Policy

All participants must submit proof-of-age documentation to the AJGA in order to apply to tournaments and/or TaylorMade-adidas Golf Qualifiers. Please direct all related questions to: ajga@ajga.org.

Proof-of-Age Submission Guidelines:

  • Please do not send any documents that need to be returned to you. All proof-of-age documentation will be kept on file with the AJGA and will not be returned.
  • No photocopies will be accepted unless notarized after the photocopy has been made.
  • No faxes will be accepted.
  • Players can login to the Player Center at ajga.org to determine if their proof-of-age documentation has been received and accepted.

Acceptable Forms of Proof-of-Age Documentation:

1. A certified proof-of-age document issued by federal, state or provincial registrars of vital statistics, or local offices, is acceptable proof-of-age, provided the document was filed, recorded, registered or issued within one (1) year of the birth of the child (i.e. birth certificate).

2. For children born abroad of a parent or parents who are U.S. citizens, any official government document issued by a U.S. federal agency or service is acceptable. These must be originals, not copies, and must refer to a filing, recording, registration, or issue date that is within one (1) year of the birth of the child.

3. A notarized photocopy of the following documents (state law permitting) will be acceptable only if the document is notarized after the photocopy has been made and the ink signature or raised stamp is visible*:

• Birth Certificate
• Valid Passport
• Valid Driver’s License
• Documents from Nos. 1 or 2 above

*If a notary is unable to notarize a photocopy, it is acceptable to write a statement on a separate sheet of paper such as, "This is a true copy." Sign the statement and have your signature notarized. Attach the two sheets together and mail to the address below.

The AJGA reserves the right to accept or reject any proof-of-age documentation not listed above.

How to obtain acceptable proof-of-age documents:

Certified birth records may be obtained from the Registrar of Vital Statistics of each state, province, or local office where the child was born, for a nominal fee. For U.S. born persons, addresses of these offices or bureaus, fees required, and other pertinent information are supplied by the United States Department of Health and Human Services. A database listing the method of obtaining birth records from any U.S. state or territory is available at the following Internet addresses:

Please send all proof-of-age documentation to:

Leadership Links

What is Leadership Links?

Leadership Links is a new program whereby the AJGA is able to further its mission to help develop young men and women by teaching charitable giving skills and service-oriented practices at an early age. This program gives juniors all the tools necessary to donate their time, talent and resources to local charities and the AJGA youth development programs.
